WAIVER WEE PLAY AT SOUTH HURON DANCE CLUB

A completed waiver by a parent/guardian is required for all minors ("Child") to play or participate in any event or activity at Wee Play At South Huron Dance Club.

In consideration of being allowed to enter and/or participate in any activities at Wee Play At South Huron Dance Club, the undersigned acknowledges, understands and agrees to the following:

1. I am the custodial parent or legal guardian of the child OR I have obtained parental authorization to act on their behalf with respect to the child/children for the purpose of executing this agreement.

2. I acknowledge that all applicable rules for participation must be followed and I assume sole responsibility for the supervision and personal safety of the child/children.

3. I, for myself and for my child/children sign this Waiver and Assumption of Risk in consideration of the opportunity to use the facility or to participate in any parties or activities at/by Wee Play At South Huron Dance Club.

4. I, for myself and for my child/children acknowledge and understand that there are dangers and risks associated with the activities at/by Wee Play At South Huron Dance Club and agree to assume all risk of personal injury.

5. I, for myself and for my child/children agree to follow the safety instructions provided and acknowledge that failure to do so may result in expulsion from Wee Play At South Huron Dance Club.

I HAVE READ THIS FORM THOROUGHLY. I UNDERSTAND THAT, BY SIGNING THIS WAIVER, I VOLUNTARILY SURRENDER CERTAIN LEGAL RIGHTS WHICH I, THE CHILD, AND OUR RESPECTIVE HEIRS, ASSIGNS, EXECUTORS, ADMINISTRATORS, PERSONAL REPRESENTATIVES, AND NEXT OF KIN MAY HAVE AGAINST THE RELEASEES, INCLUDING THE RIGHT TO SUE. I SIGN THIS DOCUMENT VOLUNTARILY AND WITHOUT INDUCEMENT.

Do you allow Wee Play at South Huron Dance Club to use any photos or videos taken today on their social media platforms? Wee Play agrees to not use the names or any personal information of any individual.

WEE PLAY RULES

• The waiver of liability/ assumption of risk must be signed before participating in any activities.
• Socks must be worn at all times, children and adults alike! No shoes, No bare feet!
• All kids must be supervised by parent(s), guardian(s) or caretaker(s). No drop-offs, unless it is a drop-and-shop event.
• No sick children or adults.
• No adults are allowed without a child in the facility;
• No toys from home;
• No reckless play, no climbing behind or on the sides of equipment, no running. You play at your own risk;
• No flips, somersaults, or other stunts!
• Enter and Exit all equipment properly, climbing over side walls will NOT be tolerated!
• Do NOT lean over the edge of equipment!
• Empty pockets and remove jewelry (we are not responsible for lost or broken items).
• We do not allow outside food – from home or from restaurants such as Tim Hortons or McDonald’s. You may bring in refillable water bottles.
• Wee Play at South Huron Dance Club reserves the right to limit the number of people that occupy the playroom. Guests may be asked to wait outside the facility in order to remain within safe occupancy limits and will be admitted once capacity allows.
• All entertainment must be coordinated in-house, with Wee Play at South Huron Dance Club. No piñatas or confetti in the facility.
